Curative Ink Hold Harmless Agreement

 I acknowledge that I am voluntarily receiving services from Curative Ink™, which may include paramedical tattooing and related procedures. I understand that these services carry inherent risks, and I have been informed of potential outcomes, contraindications, and aftercare requirements.

I hereby release, waive, and hold harmless Curative Ink™, its owners, employees, contractors, and facilities from any and all liability, claims, demands, or actions arising out of or related to any loss, injury, or complication that may occur during or after our services, except in cases of gross negligence or willful misconduct.

I confirm that I have disclosed all relevant medical information and understand that results may vary. This agreement shall remain in effect for all current and future services provided by Curative Ink™. (Last update 8/2025)
